Research Methods on Psychology – HIV & Type 2 Diabetes
Title: “Navigating Comorbidities: Exploring Self‑Management of HIV and Type 2 Diabetes”
In South Africa, the growing burden of multimorbidity, which is defined as the presence of two or more chronic health conditions, poses significant challenges for healthcare systems and patients alike. Among these, the co-existence of HIV and type 2 diabetes (T2D) became increasingly common, requiring complex management strategies to ensure optimal health outcomes. HIV, a virus transmitted through bodily fluids, requires lifelong treatment to maintain immune function and prevent complications. T2D, a metabolic disorder characterized by insufficient insulin production, demands careful monitoring of blood sugar levels, dietary adjustments, and medication adherence. Managing both conditions simultaneously presented unique challenges, including complex medication regimens, lifestyle modifications, and navigating healthcare systems.
This study explored the experiences and perspectives of patients regarding the management of HIV and T2D. Specifically, it aimed to understand the process of accessing care for these conditions and how they impacted patients’ ability to manage their health effectively.
You will be given a qualitative dataset comprising 6 interviews with participants accessing care at a public primary healthcare facility. These participants have been diagnosed with HIV and type-2 diabetes. Using the process of Braun and Clarke’s thematic analysis, you will be required to analyse the data set of 6 interviews. The dataset includes 3 interviews with male participants and 3 with female participants. The interview schedule has also been made available.
Structure of the Report:
The report should include the following;
Background [10 marks; 1-2 pages]
- A background/literature review on HIV, T2D, and accessing primary health care services at the primary health care level in South Africa.
- Based on the title of the study and the overall aim, formulate a rationale, research objectives (maximum 3), and research questions (maximum 3)
Methodology [10 marks; 1 page]
- Half a page describing the study design and population/sample.
- Half a page for the description of the analysis methodology.
Findings [20 marks; 2-3 pages]
- This section should detail the findings of the thematic analysis (codes, themes, extracts from the data).
- Consult standards for reporting qualitative findings.
Discussion [10 marks; 1-2 pages]
- A discussion of the key findings relating back to existing literature. The discussion section should interpret the findings within the study’s context, address their implications, consider limitations, and suggest future research directions.
- The discussion should include the psychological implications of living with chronic illnesses.
- Include a list of references

Background analysis
An analysis of the given scenario indicates that there is the issue of rising multi-morbidity in South Africa especially HIV and type 2 diabetes, and they also increase the treatment complexity. The issue with the public Primary Health Care is overcrowding, long waiting time and fragmented care. The patients are required to manage the glucose control and also comply with ART adherence.
Rationale – to perform an analysis of the ways in which patients navigate dual chronic conditions and Healthcare access barriers.
Objectives
- The main objectives of the study are to evaluate the experiences of patients in managing HIV and T2D.
- To explore the barriers in accessing primary Health Care.
- To make an analysis of the coping and self-management strategies adopted by patients.
Research questions
The main research questions are – how patients manage HIV and T2D, the challenges that they face in accessing Primary Care, and what the strategies they follow to improve compliance and outcomes.
Methodology
The methodology is a qualitative research design whereby 6 participants are considered from a public clinic. The sampling strategy adopted is purposive sampling because patients with both conditions are selected.
Analysis method – the analysis is carried out using Braun and Clarke thematic analysis method by applying coding, theme development, and interpretation.
Findings
The findings from this study are categorized into different themes as follows:
Theme one – treatment burden – it is evaluated that the multiple medications leads to confusion and fatigue, as patients are required to take too many pills at different times.
Theme two – healthcare access barriers – the findings lead to identification that the patients have to undergo long queues and there are medication shortages. Because of long queues, there are missed appointments which affect both conditions.
Theme three – psychological stress – the findings leads to identification that the patients experience anxiety from managing chronic illnesses, and they always have a fear of complication and stigma.
Theme four – coping strategies – the coping strategies followed are routine building, family support, and reliance on peers advice instead of professionals.
Discussion
The findings from the analysis properly align with the literature review findings on multiple morbidity burden in South Africa. The main issue identified is the inappropriate health system as it does not support dual care. The psychological impact is stress, burnout and reduced motivation.
Implication
From the findings it is identified that there is a need for integrated chronic care models, and improved patient education and counseling is a necessity.
Limitation
The limitation of the given study is the small sample population, and there is a single clinic considered in selecting samples which limits generalisability.
Future research
The future research should focus on multisite studies and community-based support systems.
